HL7 v2 data types

Every field in an HL7 message has a data type, and it settles one question: does this field hold a single value, or several values packed together?

The two kinds

A primitive holds one value. A date of birth is a date; a haemoglobin result is a number. The field holds it as it stands, and there is nothing to split.

A composite holds several values that belong together, separated by a caret. A name is a family name and a given name and a suffix, and each of those parts is itself a primitive.

PID-5, a patient name. Data type XPN. Four of its fourteen components are filled in: family name, given name, middle name, and the code that says this is her legal name.

That is the whole of it. There are 90 data types, 12primitive and 78 composite. Every composite is built, eventually, out of the primitives below.

Primitives

12 of them, and these 8 cover almost everything you will read. Open one to see what it holds.

DTDateholds one value

A date with no time, written as YYYYMMDD. Shorter forms are legal: a year alone, or a year and month.

A four- or six-character value is valid. Code that assumes eight characters breaks on a patient who only knows their year of birth.

DT is a primitive: one value, no internal structure, nothing to split on. The standard states a maximum length of 8 and calls the value Date.

A short code from a list the standard fixes. An HL7 table sits behind it, and a value outside that list is an error.

ID and IS look identical in a message. The difference is who owns the list, and it decides whether an unrecognised code is a bug or a local extension.

ID is a primitive: one value, no internal structure, nothing to split on. The standard states a maximum length of no stated limit and calls the value coded value for HL7 defined tables.

A short code from a list the standard only suggests. A user-defined table sits behind it, and a site is free to use its own values.

MED

the medical service, in PV1-10

A validator that rejects codes outside the published list will reject real, conformant traffic. Suggested means suggested.

IS is a primitive: one value, no internal structure, nothing to split on. The standard states a maximum length of no stated limit and calls the value coded value for user-defined tables.

NMNumericholds one value

A number, written as digits with an optional sign and decimal point. No units and no thousands separators.

The units live in a different field. A number stored without OBX-6 beside it cannot be interpreted: 11.8 g/dL and 11.8 g/L are not the same measurement.

NM is a primitive: one value, no internal structure, nothing to split on. The standard states a maximum length of 16 and calls the value Numeric.

SISequence IDholds one value

A small positive number that counts repeating segments within one message. It is a position, not an identifier.

It restarts. OBX-1 counts from 1 again under each OBR, so it is unique within a group and not within the message.

SI is a primitive: one value, no internal structure, nothing to split on. The standard states a maximum length of 4 and calls the value Sequence ID.

STString Dataholds one value

A short piece of text, used as it stands. No internal structure, nothing to split on, up to 199 characters.

A delimiter inside the text arrives escaped. A value of O\S\Brien is one name containing an apostrophe, not two components.

ST is a primitive: one value, no internal structure, nothing to split on. The standard states a maximum length of 199 and calls the value String Data.

A time of day with no date, written as HHMM, optionally to the second and fraction, optionally with a timezone offset.

Text meant for a person to read, long enough to hold a paragraph. ST holds a label; TX holds a sentence.

A long report arrives as several repetitions or several OBX segments, each one a line. Joining them without a line break runs the whole report together.

78 of them, which is where the caret comes from. These 10 are the ones you meet in an ordinary message.

A code plus the name of the thing it codes plus which coding system it came from. Then optionally the same three again, from a second system.

The three components after the first three are a second coding of the same concept, usually the sending system's local code. Both are worth keeping: one for interoperability, one for reconciling against the sender.

An identifier plus who issued it. That second part is what makes it meaningful, because an identifier on its own is only unique within the organisation that assigned it.

Matching on the identifier alone merges patients. Two hospitals will both issue an MRN of 1000345678 and they are not the same person; the assigning authority in component 4 is what tells them apart.

Sites fill this in inconsistently, and a common local habit is to put the whole location in component 1 and leave the rest empty. Read all four components and reassemble; parsing component 1 as free text will work at one site and fail at the next.
